Population Council Research that makes a difference

Overview

In 1979, the Population Council established an office in Mexico City as its regional office for Latin America and the Caribbean. Today it functions administratively as a country office, but Mexico-based Council investigators work throughout the region.

As a middle-income country, Mexico has celebrated considerable improvements recently in health, although stubborn inequities according to socioeconomic status and region remain. Similarly, Mexico continues to address sexual and reproductive health challenges. For example, although the total fertility rate has reached a plateau at slightly above replacement level (2.2), substantial unmet need for contraceptives persists, especially among the poor; women continue to die during pregnancy despite near-universal access to institutional delivery; and although elective first-trimester abortion is legal in Mexico City, it is highly restricted outside the capital, causing significant abortion-related morbidity and mortality. Through social science and epidemiological and public health research, the Council has created a substantial evidence base around these critical topics to help shape policies and programs that help the most vulnerable Mexicans.

Program focus

Although the Mexico office is committed to seeking new research opportunities in all three Council Programs (HIV and AIDS; Poverty, Gender, and Youth [PGY]; and Reproductive Health) most of our recent and ongoing research has a reproductive health focus with clear links to HIV and AIDS and Poverty, Gender, and Youth.

Building evidence for safe and legal abortion. The Council has investigated sexual and reproductive health in Mexico and LAC for more than 30 years, including pioneering work on abortion incidence, public opinion, and, since the passage of the watershed abortion reform in Mexico City in April 2007, improving access to safe and legal abortion in the capital. Since 2000, we have lead the research agenda within an innovative five-member consortium of nongovernmental organizations that work tirelessly to position sexual and reproductive rights squarely on the public agenda. We also work in partnership with the Mexico City Ministry of Health to conduct studies aimed at strengthening access to and the quality of legal abortion services. Our progress is the result of comprehensive, synergistic, and responsive strategies that focus on: (1) quantifying and demonstrating the public health burden of unsafe abortion; (2) showcasing its substantial economic and social costs; and (3) documenting public opinion favorable to safe termination of pregnancy. 

Making pregnancy safe. Although Mexico’s maternal mortality rate (MMR) has improved in recent years, this decline has not kept pace with the country’s overall socioeconomic development. With an MMR of 60 maternal deaths per 100,000 live births, much work remains. For example, in order to reach Millennium Development Goal number 5 aimed at improving national MMRs, Mexico must reduce its current MMR by 40 percent by 2015. The main causes of maternal death are hypertensive disorders, hemorrhage, and other direct causes. Through interinstitutional collaborative agreements, the Council works closely with federal and several state-level ministries of health to improve the adoption of evidence-based practices in obstetric care. We will soon expand our maternal health research to include the support of high-quality obstetric services. Research in two border states (Chihuahua on the United States border and Yucatán on the Belize/Guatemala borders) aims to help women advocate for their own health care, particularly when interacting with physicians and nurses. Also, we will train health-care professionals in ways to offer women high-quality care during pregnancy and delivery. Lastly, we will continue collaborating with our Guatemala office colleagues on their maternal health activities.

Strengthening RH interprogrammatic links with HIV and AIDS and PGY. Our RH research in Mexico intersects with key priorities of the HIV/AIDS and PGY programs, and we seek opportunities to leverage the Council’s global experience across programs. Good national health indicators often mask pockets of marginalization among the most vulnerable individuals. Using lessons from the Council’s global research on safe spaces, we are documenting the health needs and experiences of vulnerable young people and currently are evaluating a program to empower vulnerable girls in Oaxaca State. Similarly, although Mexico’s HIV prevalence is relatively low and its response to the pandemic is strong, certain groups remain underserved and underrepresented because they do not fit into the traditional high-risk categories (injecting drug users and men who have sex with men). Consequently, we focus our integrated HIV research on vulnerable women living with HIV and on migrants.

Looking forward. Council researchers in Mexico exploring new areas of research to better understand the association between urbanization, poverty, and health, especially in the extended capital city. We will also begin new research with HIV-positive women, building on the previous studies discussed above.

Highlights of recent and ongoing research

    Reproductive Health

      • A 2008 study of national abortion incidence in Mexico revealed a 33 percent increase in induced abortion since 1994 and no improvement in abortion-related complications. In 2011, ongoing research will highlight the public health burden of unsafe abortion among the most vulnerable Mexicans by age and state.
      • A 2009 study of the total costs to the public health system of treating complications due to unsafe abortion in Mexico City versus offering safe early abortion services, and a 2011 study will explore the social costs of abortion.
      • The Council is a forerunner in the region in terms of conducting high-quality opinion surveys about induced abortion at the national and state levels. Examples of such studies include: (1) consecutive representative surveys among Mexico City residents before and after legalization of elective first-trimester abortion; our studies indicate increased public support for the Mexico City law from 38 percent in 2007 to 73 percent in 2009; and (2) several additional opinion studies among lawyers (2009); women (2007, 2009); young people (2004); and physicians (2002), most in Mexico, but also in Brazil, Honduras, and Jamaica.
      • Researchers have formed technical partnerships with the Mexico City MOH on data monitoring and descriptive analysis of women who obtain legal abortions, including clarifying women's and physicians' experiences, and other related studies (since 2007 and ongoing).
      • Since 2001, the Council’s Mexico office has hosted three major academic conferences in LAC on the public health impact of unwanted pregnancy and abortion. The 2008 meeting drew more than 250 colleagues from around the globe. In 2011, the event will highlight experiences from Central American countries.
      • Nationally, and in Mexico City and Oaxaca (Mexico’s poorest state) specifically, we are systematically documenting the use of magnesium sulfate for the prevention and treatment of severe pre-eclampsia and eclampsia in public-sector services and barriers to its use.
      • A 2011 project in the state of Yucatán will assess the feasibility of introducing a rapid test for pregnant women's high levels of albumin and a Doppler study measuring women's uterine artery, a gold standard in early pre-eclampsia diagnosis.
      • In Oaxaca, we are assisting a local academic institution to evaluate the impact of an obstetric emergencies training course for obstetricians and gynecologists.
      • With the Federal and Mexico City MOHs, the Council is completing an extensive review of the medical charts of women who died during pregnancy in the past three years.
      • We work closely with our colleagues in the Guatemala office and with a Guatemalan network of indigenous women (CONAVIGUA) to document the experience of indigenous women who use maternity waiting homes and to train peer health promoters on better integration of traditional and Western birthing practices.

    Reproductive Health and Poverty, Gender, and Youth

      • A three-year collaboration in Oaxaca is assessing the impact of an innovative, rights-based leadership training program on young indigenous women and their peers. We are also conducting a census survey on SRH needs and gender equity among young people.
      • We are conducting studies aimed at understanding the role of men in abortion stigma and postabortion contraceptive use in Mexico City.
      • With colleagues at the World Health Organization in 2009, we carried out an important secondary study for forthcoming operations research to introduce the HPV vaccine as part of an integrated package of adolescent health services in Mexico, Panama, and Peru.
      • Through membership on the Harvard School of Public Health's Cross-national Initiative on Place, Migration, and Health (CIPMH), we are committed to understanding the links between migration processes and the health of Mexican immigrants/migrants, their families, and their sending/receiving communities.

    Reproductive Health and HIV

      • A recently completed qualitative study in four Mexican cities assessed the SRH experiences and barriers to care from the perspectives of women living with HIV and a sample of their healthcare providers and revealed a lack of SRH and HIV service integration and persistent HIV-related stigma in the health system. In December 2010, together with local partners, we will convene a meeting with healthcare officials, study participants, and advocates to create strategies for improving health care for HIV-positive women.
      • A study among Mexican migrant men residing in Texas found moderate reported risk factors for HIV infection and substance abuse and lends evidence for future studies with migrants.

Assessing the Sexual and Reproductive Health Needs and Experiences of Indigenous Youth in Oaxaca (more)
