This page includes the full list of surveys that have been listed  alphabetically by primary  organization (there are often other partners on the study as well) and scales listed alphabetically by title. These instruments are provided as either  Microsoft Word or PDF files or there is a link to an external website where they are available.

Next to the title of the instrument is a link to a brief survey summary, which contains information on where the instrument has been used and with which populations, as well as on the validity and reliability of the instrument, when information was available.

Most of the instruments are in English, although we have included some foreign language surveys where indicated.  Most of the instruments are closed-ended quantitative questionnaires, but we have included some qualitative guides as well.  

Further information on any copyright or permission procedures is available at the website where the instrument is located.

All Horizons instruments are available for public review.
