Pay for Performance (P4P) Operations Research Study
Following a successful feasibility study on introducing a P4P financing scheme, the Population Council is now conducting operations research to test the potential of this service-delivery model to increase utilization of maternal, newborn, and child health care through performance-based payment to providers.
Lack of skilled birth attendance is one of the key contributing factors to high maternal and neonatal mortality in Bangladesh. Limited utilization of maternal, newborn, and postnatal care by trained professionals is influenced by both demand- and supply-side barriers. As a follow-up to a study funded by the Center for Global Development on the feasibility of introducing a Pay for Performance financing mechanism in Bangladesh, the Council is now assisting in an operations research study funded by UNICEF to develop and test the service-delivery model to increase utilization of maternal, newborn, and child health care, as well as postpartum contraceptive services, through performance-based payment to service providers.
The approach will involve a combination of supply- and demand-generated factors. On the supply side, a system to pay service providers in the public sector based on performance will be developed. Financial incentives will be offered at the institutional level and directed toward improving health outcomes by motivating and encouraging providers to be accountable and responsive to the patients they serve. On the demand side, financial assistance will be provided to poor pregnant women for medicine and transport to services at health facilities. To maximize potential scale-up, the project will be implemented in districts where UNICEF/Bangladesh and BRAC have partnered on effective maternal, newborn, and child health interventions.
If results of the operations research project demonstrate feasibility and effectiveness, one or both interventions will be scaled up to a larger but manageable number of facilities. This controlled scaling-up will be systematically documented to understand critical system-level elements that must be addressed before recommendations can be made about introducing the intervention to the national program.
