Population Council Research that makes a difference

Each year, 60 percent of maternal deaths (and 75 percent of newborn deaths) occur in the first seven days postpartum. Moreover, many new mothers continue to have critical health needs throughout the postpartum period. Even though the key services of a postpartum care package are known and often are available in clinics, fewer than 30 percent of women in developing countries seek or receive care or medical follow-up during the postpartum period. The postpartum needs of women living with HIV are particularly acute and underserved.

Population Council focus

Through research and technical assistance, the Council is developing sustainable means for increasing access to and use of postpartum care by strengthening and increasing the range of existing services, decentralizing services to the community level, and testing innovative strategies to improve service effectiveness. In addition, the Council seeks to understand and reduce the gap between availability and use of services by educating and mobilizing communities on the importance of postpartum care. The Council's programs in this area focus on:

    • Increasing access to and use of postpartum care
    • Decentralizing services to the community level
    • Testing strategies to improve service effectiveness
    • Educating and mobilizing

Population Council impact

    • In Mexico, the Council conducted a large study of the verbal autopsy methodology that demonstrated that verbal autopsy has inherent limitations in identifying the medical causes of death and reconfirms that mortality/morbidity among poor women with low access to care is substantially higher than among wealthier women.
    • An assessment of postpartum care in low socioeconomic settlements of Pakistan, where few women attend the recommended six weekly postpartum visits, indicated that spousal opposition was the main reason for not opting to delay the next pregnancy and use contraception.
    • Expanding service delivery (ESD) projects in India and Egypt are scaling up demonstrated models of community-level postpartum care that use community mobilization focusing on male partners and leaders and on promoting birth-spacing messages to increase understanding of the importance of postpartum care.
    • Council activities in Kenya, South Africa, and Swaziland are developing and evaluating the effectiveness of integrated packages of postpartum care that systematically offer women services at one to two weeks, six weeks, and six months. These packages address the needs of new mothers living with and without HIV.

      Council researchers worked with Kenya's Division of Reproductive Health and ACCESS-FP/JHPIEGO to develop and introduce a strengthened postnatal care package for Kenyan women in selected facilities with maternity units.
