Population Council Research that makes a difference

Unsafe abortion continues to afflict the lives of many women, mostly those living in developing countries. In many countries, the procedure is either entirely prohibited or permitted only under certain circumstances. Independent of its legality, however, abortion continues to occur, with nearly half of abortions performed by unskilled providers or in unsafe conditions.

According to the World Health Organization, 13 percent of all maternal deaths are due to unsafe abortion. In 2008, an estimated 21.6 million unsafe abortions took place worldwide; in developing countries, 47,000 unsafe abortions resulted in maternal deaths.

An estimated 210 million women become pregnant each year in the developing world, and approximately 15–20 percent of their pregnancies end in abortion, whether spontaneous or induced. Complications arising from spontaneous (miscarriage) and incomplete induced abortion are widely recognized as a major public health concern, both in countries where abortion is illegal or legal with significant restrictions. For example, even in settings in which it is legally available, access to safe services is limited for many—especially women who are poor and live in rural areas. Poor quality of care, inadequate provider–client communication, lack of counseling on medical procedures, and limited use of contraception after the procedure are additional problems experienced by many women. Furthermore, stigma remains a large barrier for many clients and providers, regardless of the legal status of abortion. Postabortion care (PAC) is an integrated service that includes emergency treatment of complications, counseling and provision of contraceptives and STI/HIV services, provider training, and community empowerment through awareness-raising and social mobilization.

Population Council focus

Population Council research aims to save women's lives by reducing the morbidity and mortality caused by unsafe abortion. In countries where abortion is legal under a range of circumstances, we achieve this goal by helping national counterparts make services safer, more affordable, and more widely available.

Council research involves a range of community- and clinic-based studies focused on improving the availability, accessibility, safety, and quality of abortion services in settings where abortion is legal; exploring determinants of abortion and care-seeking behaviors; investigating knowledge and attitudes about abortion services among clients, men, and providers; and documenting the experiences of the most vulnerable, including young and unmarried women. The Council designs and implements innovative research and evidence-based advocacy to ensure that abortion services, where legally available, are widely accessible, safely provided, and affordable to all who need them. Council research also focuses on the reduction of stigma surrounding abortion. The Council is a member of regional and national consortiums on safe abortion in several settings, which are aimed to improving access and quality particularly for poor and rural women.

Where abortion is not legal, the Council aims to both document the public health and cost burden of unsafe abortion (e.g., by testing innovative ways to measure abortion incidence or prevalence rates)  and improve the availability of postabortion care (PAC) services. Through operations research and technical assistance, the Council evaluates the feasibility, quality, and cost-effectiveness of strengthening existing or developing innovative PAC strategies. The Council’s FRONTIERS program collaborated with a diverse group of organizations and stakeholders in Africa, Asia, and Latin America to design and implement interventions for PAC. Assessments of these interventions have communicated valuable lessons and patterns from regional experiences, which help to build national capacity for the utilization of PAC services through program and policy development. The Council is a member of the PAC Consortium, which endorses a model for care that combines community and service provider partnerships, counseling, treatment (including vacuum aspiration, infection prevention, and pain management), family planning and contraceptive services, and links to reproductive and other health services.

Population Council impact

    • In India, where abortion is legal but access is limited, the Council has built evidence to support and advocate for expanding the types of providers trained to conduct manual vacuum aspiration and medical abortion thereby expanding women’s access to these services and providing the evidence to support the amendment of the MTP (abortion) Act.
    • With partners in Ghana, the Council is seeking to reduce the number of unsafe abortions through increasing access to contraceptive services and to skilled provision of comprehensive abortion care services.
    • The Council contributed to monumental policy change in 2007 in Mexico City, which has enabled women from within the capital but also from states where it remains restricted to receive safe first-trimester abortion and postabortion care from qualified providers.
    • The Council supported the Ministry of Health in Egypt to develop service-delivery guidelines for providing postabortion care. These have subsequently been shared with stakeholders in Jordan, Sudan, and Yemen.
    • In Senegal and Burkina Faso, a pilot operations research project initiated by the Council and Jhpiego helped lead to the integration of PAC in to the national emergency obstetric care strategy (EmOC) in each country. Stakeholders in Guinea and Niger and several other francophone countries used these experiences to advocate within their own ministries of health.
    • In India, the Council is a member of the consortium for safe abortion services, a consortium that has demonstrated effective models of comprehensive abortion care in rural settings, and advocated for policy change.
